Prospect Elementary Kindergarten Registration

2011 – 2012

Children who are five (5) years old by September 30, and have received the required immunizations:

5 DPT                         4 Polio                         2 MMR

Hepatitis B Series (3 doses)                           Varicella (2 doses)

*The Physician’s Report (in the packet) must be filled out by a physician & returned by the first day of school

1.  Pick up packets at Prospect Elementary, Girard Free Library, or the Superintendent’s Office

February 14 – March 4

2.     Return completed packets February 28 – March 11 at Prospect Elementary or

Wednesday March 9 from 4:00 pm – 7:00 pm

(Please have all required documentation)

Birth Certificate                       Social Security Card

Complete Immunizations                  Custody Papers

Food Stamp/Medicaid Card    IEP/MFE (if applicable)

Proof of Residency:       Mortgage/bank payment book/Lease (notarized)

Utility bill/s                    2 Pieces of business mail

Valid Ohio Driver’s License

(only completed packets will be accepted & a screening scheduled)

3.  Screening April 6^th & April 13^th, & May 4^th 2011 at the Girard Free Library (by appointment)

Prospect Elementary MUST have all required documentation for placement in a class for the

2011 – 2012 school year

Please Note:         Children need not by present for registration

A Kindergarten Screening Evaluation & visitation will be scheduled during registration

• A Girard book bag with supplies will be given to each student who registers and completes the screening on one of the three dates above.
